We will mail you our designated "Request Form for Disclosure of Personal Information.After completing all required fields on our designated "Request Form for Disclosure of Personal Information" and affixing your seal, please send it via registered mail to the address below, along with documents necessary to verify your identity as the individual or authorized representative. If you are requesting "Notification or Disclosure of the Purpose of Use of Personal Information Subject to Disclosure," please also include the handling fee.

Please submit one of the following: driver's license, health insurance card, passport, certificate of alien registration, copy of student ID card, or seal registration certificate with registered seal. When submitting these documents, please black out or make unreadable any sections containing sensitive information not necessary for identity verification (example: permanent domicile on driver's license).Identity verification can also be conducted by telephone. If you prefer this method, please contact the Personal Information Consultation Desk Manager. After asking you for information that can verify your identity by phone (such as date of birth), we will contact you back by phone at the contact number we have on record for you.

Please submit a power of attorney indicating voluntary agency and either a certificate of seal registration for the principal whose seal is affixed to the power of attorney, or identification documents for the voluntary agent.

When Requesting Through a Legal Representative

Please submit one of the following documents proving legal representation (family register transcript, family register extract, family court certificate, certificate of registered matters, or other documents certifying the qualification (issued within 30 days prior to the date of the disclosure request)), together with identification documents for the legal representative themselves.

Please enclose postal stamps worth 500 yen in the registered mail when sending the "Request Form for Disclosure of Personal Information."

Cases Where We Cannot Respond to Requests for Disclosure, etc.

If any of the following applies, we will not be able to respond to your request for disclosure, etc. If we decide not to disclose, we will notify you accordingly. Please note that the prescribed fee will be charged even in cases of non-disclosure.

• When the identity of the individual cannot be confirmed
• When the authority of a representative cannot be confirmed in applications made by a proxy
• When there are deficiencies in the prescribed application documents
• When the personal information held by our company cannot be identified based on the content of the request
• When there is a risk of harming the life, body, property, or other rights and interests of the individual or a third party
• When it is necessary to cooperate with a national government agency or local public entity in performing duties stipulated by laws and regulations, and notifying the individual of the purpose of use or making it public would likely hinder the performance of such duties
• When there is a risk of significantly impeding the proper implementation of the company's operations
• When it would violate laws and regulations
• When the purpose of use is deemed clearly evident from the circumstances of acquisition in a request for notification of the purpose of use
• When there is a risk of harming the Company's rights or legitimate interests in a request for notification of the purpose of use
